Job Description

We seek a dedicated and compassionate Registered Nurse (RN) to join our home health team. In this role, you will be at the forefront of delivering high-quality care to our patients. You will work closely with patients, their families, and other healthcare professionals to create and implement comprehensive care plans that meet each patient's needs.

Responsibilities

• Conduct initial assessments of new patients and develop individualized care plans.
• Provide direct patient care by the patient's individualized care plan.
• Provide skilled nursing care, including medication administration, wound care, and monitoring of vital signs.
• Collaborate with a multidisciplinary team to facilitate integrated and comprehensive care.
• Assess, monitor, document, and report the progress of the patient's health and condition.
• Provide emotional support and compassionate care to patients and their families.
• Educate patients and their families on disease management, self-care, and end-of-life planning.
• Maintain accurate and timely documentation of patient care.
• Adhere to all company policies, procedures, and regulatory requirements.
• Ability to be on call as needed.

Qualifications

• Current and unencumbered RN license in the state of New Mexico.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work independently and manage time effectively.
• Basic computer skills, including experience with electronic medical records (EMR) systems.
• CPR certification and valid driver's license with reliable transportation.
• Strong problem-solving and decision-making skills.
• Compassionate and empathetic demeanor with a strong dedication to patient care.

Pay: $43.00 - $129.00 per hour
