• Must be able to reliably commute to all areas of Solano County
• Collaborate with other team members to ensure each client completes an initial intake and needs assessment.
• Determine the nature of an MCP member's situation by interviewing the member and evaluating their medical, psychological, emotional, and social information.
• Establish a course of action by setting goals with the member and exploring various options.
• Obtain assistance for the member by referring them to community resources, arranging appointments, and collaborating with other agencies.
• Maintain case records through accurate documentation of a member's situation and their actions.
• Monitor and evaluate member progress along with care plans, goals, interventions, and modifications as needed.
• Communicate and coordinate services with the member and their care team — including their Primary Care
• Facilitate member access to behavioral health services, social services, and community-based services as needed.
• Assist youths in determining their personal goals and plans to achieve those goals through an Individual Action Plan (IAP).
• Provide clients with housing stabilization services — including classes focused on money management, life skills, transportation access, understanding their lease, self-advocacy, and other topics that meet their needs.
• Participate in outreach efforts to reach clients experiencing homelessness.
• Cultivate relationships with landlords and other housing providers to build a portfolio of housing options for clients.
• Enter data in required databases. Compile and submit reports that track client status and support services provided.
• Maintain accurate client case files of all required eligibility documents.

Pay: $20.00 - $25.00 per hour
