RESPONSIBILITIES:

• Coordinate and facilitate patient care through assessment, evaluation, planning, and implementation
• Communicate patient needs to a variety of care team members and follow up accordingly
• Work collaboratively with patients, families, physicians, and nurses to ensure high quality care
• Infection Control: Track Covid Cases for facility, provide education to all staff regarding infection control: Monitor trends of various types of infections
• Imunization tracking
• assist with med room/med cart audits weekly and follow up with pharmacy as needed.

QUALIFICATIONS:

• Must have active Nursing License (RN/LPN)
