Education : Master's degree
License Requirements - Nurse Practitioner
Nurse Practitioners
- State License
- BLS and ACLS
- Board Certified by the American Nurses Credentialing Center (ANCC)
- Minimum of five years of experience
- DEA
- NPI
- OIG search results
Drug Screen : Yes
Tdap required of all contract personnel (one dose).
If not vaccinated, COVID and Influenza exemptions are required (masks must be worn if not vaccinated).
Proof of malpractice insurance is required
Social Security Numbers, dates of birth, place of birth (city, state and country) (section B4.4.b)
