Lakeshore Community Services is Hiring a LPN Near Erie, PA

LPN

ESSENTIAL JOB FUNCTIONS

  • Complete quarterly nursing assessments and consumer-related paperwork.
  • Assist in identifying consumers with complicated medical or psychiatric needs and provide appropriate level of nursing care within scope of program.
  • Respond to consumer-specific needs such as teaching and support of medical conditions.
  • Meet periodically with Treatment Team and program staff and management to provide and receive information about consumer progress.
  • Maintain a consumer record and records system that would meet current regulations and HIPAA requirements.
  • Communicate to staff when consumers have specific needs and changes.
  • Coordinate and provide training for staff /consumers related to the needs of the consumers.
  • Identify, respond to and follow-up with any medication variances that occur.
  • Reports to or works in conjunction with waiver, RN.
  • Assist staff and/or consumers with obtaining and following up with doctors/clinics refills when indicated.
  • Obtain clarification from other providers regarding discrepancies and concerns.
  • Review and approve medicals reports in Evolve.
  • Offer and provide teaching and support related to any changes in medications.
  • Participate in discharge planning when consumers enter a facility. Obtain discharge papers and medical records in a timely manner.
  • Provide follow-up care to consumers after discharge/release from any facility.
  • Maintain effective, cooperative communication within program as well as internal and external programs.
  • Provide on-call support during non-business hours to staff reporting any consumer medication or medical concerns and assessing risk.
  • Will attend in-service training sessions as required.
  • Other responsibilities as assigned.
  • Review HRSTS
  • Monitor consumer appointments are completed on time.

Job Type: Full-time

Salary: $21.89 per hour
