Social Model Recovery Systems, Inc is Hiring a Primary Counselor (Omni) Near El Monte, CA
- Role model appropriate healthy and ethical behaviors in all interactions, including HIPAA and Confidentiality standards.
- Maintain a primary caseload of participants, assisting and advocating for them in meeting their unique needs within the program structure.
- Develop a treatment plan for each participant on the caseload and evaluate their progress in meeting the goals and objectives within the treatment plan.
- Update the individual treatment plans, as appropriate, in a timely manner.
- Document the services provided according to standards set by the Agency and applicable County, State, and Federal regulations. Documentation should be clear, concise, and completed in a timely manner.
- If determined by the Program Director, maintain and update program curricula.
- Facilitate full utilization of service units, including Case Management, as directed by the supervisor.
- Assist with participant’s benefits (e.g. DPSS, SSI, Medi-Cal) application(s).
- Familiarity and adherence to all regulatory expectations and ethical standards and Agency policies and procedures, including the Agency’s Core Beliefs.
- Maintain constructive relationships with family members, community members and organizations, human services agencies, and other professionals.
- Assist in ensuring a safe and alcohol/drug-free environment.
- Perform all other duties as assigned.
