Overview:
We are seeking a dedicated Care Manager to join our team. The ideal candidate will have experience in crisis management, workforce development, and addiction counseling. Duties:
- Provide case management services to clients, including assessment, goal setting, and monitoring progress
- Conduct crisis intervention when necessary and provide support during challenging situations
- Develop individualized service plans based on clients' needs and goals
- Assist clients in accessing community resources and services such as childcare, Section 8 housing, and addiction counseling
- Supervise and support clients in achieving self-sufficiency and independence
- Collaborate with internal teams and external agencies to ensure comprehensive care for clients
Requirements:
- Bachelor's degree in Social Work, Psychology, or related field (preferred)
- Proven experience in case management, child welfare, or related field
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Knowledge of motivational interviewing techniques
- Familiarity with supervising and mentoring individuals
