SpringVale Health Centers is Hiring a Behavioral Health Assessment Specialist Near Dover, OH

***SIGNING BONUS $1,200***

SpringVale Health Centers is seeking a full-time Behavioral Health Intake Specialist to provide initial diagnostic assessment, short-term interventions, and triage of services for individuals referred for services. Based on identified needs, the Intake Specialist shall initiate the referral process and ensure the successful linkage of person served to the needed services. Independently licensed provider is preferred.

Responsibilities:

  • Provides Diagnostic Assessment and individualized service planning utilizing appropriate clinical skills and training. Constructs a concise narrative summary and appropriately uses information to establish an appropriate diagnostic impression.
  • Provides counseling services utilizing appropriate clinical skills and clinically accepted evidence-based treatment approaches. Intervenes using appropriate crisis intervention skills and anticipates and prevents crisis situations from escalating.
  • Completes all required Diagnostic Assessment forms and completes the assessment updates within the timeframes established by agency policy and procedure. Participates in case reviews as required.
  • Serve as agency liaison by maintaining collaborative efforts to ensure that timely and quality services are provided to individuals referred to the agency. Established therapeutic rapport with individuals, while defining boundaries.
  • Demonstrates professionalism with individuals served and other community professionals/agencies while providing the highest level of service.

Minimum Requirements:

Master’s degree in counseling, social work or other mental health related field with current Ohio licensure as an LPC, LPCC, LSW or LISW. Independently licensed provider is preferred.
