Hospice Registered Nurse
Responsibilities:
- Evaluates and maintains an ongoing assessment of individuals and their families to identify Hospice requirements
- Manages patient admissions and prepares the initial patient/family Care Plan
- Utilizes health assessment information to establish nursing diagnoses
- Collaborates with patients, families, and a diverse team of hospice professionals to coordinate services
Requirements:
- Active and valid Registered Nurse (RN) license
- Completion of an RN Diploma, Associate's, or B.S. degree in nursing
- At least 1 year of prior experience in the Hospice field
- Exceptional skills in observation, problem-solving, and communication
- Displays compassion and upholds a high level of integrity
