DHCHD Brand is Hiring a Mental Health Coordinator Near Dalhart, TX

Dallam Hartley Counties Hospital District invites you to become an integral part of our team! We are looking for a Mental Health Coordinator to join our behavioral health team. Under the supervision of the Behavioral Health Director, the Mental Health Coordiantor provides direct patient care to senior clients, assists in clerical duties, maintains medical records, assists with tele-med appointments, and assists the behavioral health professionals with daily tasks. Additionally, the Mental Health Coordinator transports patients to and from the program as needed. This position requires a highly confidential, positive, service-oriented individual who enjoys working with older and senior adults.

Essential Duties and Responsibilities 

  • Performs clerical functions to facilitate the program, including but not limited to:
    • Contacting patients daily via telephone to coordinate attendance.
    • Opening/distributing/handling daily mail.
    • Review and approve daily/monthly census, billing and audit reports.
    • All paperwork associated with admitting and/or discharging patients.
    • Prepare weekly schedules for patients.
    • Update patient charts daily, weekly, monthly; preparing data assessments and verifying accuracy/validity of releases and consents.
    • Obtain quarterly patient surveys and complete reports associated with the surveys; DMARS.
    • Verify insurance at admission and regularly throughout the patients’ attendance in the program. Make copies of insurance card/information.
    • Monitor patients’ doctor visits and update chart with new information.
    • Obtain signatures as required.
    • Reconcile billing corrections as needed and resubmit.
    • Lead “flash” meetings daily.
    • Any and all scanning, faxing, and filing associated with the clerical functions required to facilitate the program.
  • Performs service duties associated with patient care, including but not limited to:
    • Preparing snacks, meals, drinks for patients.
    • Facilitating personal needs of patients (Toileting, etc.).
    • Assisting patients to an outside break area if needed.
    • Transport patients to/from program either by wheelchair or automobile.
  • Recheck all information sheets for completeness and accuracy.
  • Provide patient education as directed by provider.
  • Maintains confidentiality while working with confidential matters on a daily basis.
  • Follows safety guidelines and policies for DHCHD.
  • Any other assignments as assigned by director.
