Description

ROLE TYPE: PRN to Full-Time

Exalt Health, an acute rehabilitation hospital, provides an intensive rehabilitation program, and admitted patients must be able to tolerate three hours of intense rehabilitation services per day. Focused on caring for patients with complex rehabilitative needs such as stroke, spinal cord injury, brain injury, head trauma, medically debilitation conditions, neurological disorders, cardio-pulmonary amputations, orthopedic injuries, and multiple major traumas.

Position Summary

Responsible for participating in our interdisciplinary team, providing specialized wound care to adult and geriatric patients recovering from various conditions. You will leverage your in-depth knowledge of wound healing principles to assess, treat, and monitor a variety of wounds, promoting optimal healing and preventing complications.

Essential Duties and Responsibilities

Manages facility wound care program including standards of care and practice related to wound, ostomy, and continence patient care needs. In addition, will provide direct patient care and assistance to staff nurses, and act as consultative service to affiliated healthcare agencies.

  • Conduct comprehensive wound assessments, including identification of wound type, cause, and healing stage.
  • Develop and implement individualized wound care plans in collaboration with physicians and other healthcare professionals.
  • Provide skilled wound care interventions, including debridement, dressing changes, and negative pressure wound therapy.
  • Monitor wound healing progress, document changes, and adjust treatment plans as needed.
  • Educate patients and families on proper wound care techniques and self-management strategies.
  • Collaborate with the interdisciplinary team to optimize patient care and discharge planning.
  • Maintain up-to-date knowledge of best practices in wound care through ongoing education.

Requirements

Minimum Qualifications 

(Knowledge, Skills, and Abilities)

Licenses or Certifications

  • Current, unencumbered registered nurse license to practice professional nursing in the state where currently practicing.
  • Professional certification WOCN, WCC or CWS, or obtain certification within 12 months of employment.
  • Current BLS via the American Heart Association or American Red Cross is required.
  • Current ACLS certification via the American Heart Association or American Red Cross is a plus. 

Education, Training, and Experience

  • Graduate of an accredited school of nursing.
  • 1 year of direct patient care nursing experience required. 
  • 2 years of direct wound care nursing experience required.
  • Knowledge in wound care consistent with AHCPR, WOCN, and CDC guidelines.
  • Knowledge of wound debridement as indicated within the level and scope of practice.
  • Strong clinical skills in assessment, medication administration, and patient care.
  • Excellent communication, interpersonal, and collaboration skills.
  • Compassionate and patient-centered approach to care.
  • Ability to work effectively in a fast-paced and team-oriented environment.
  • Passion for patient-centered care and a commitment to rehabilitation services.
