JOAK American Homes - Macomb is a contract provider for Macomb County Community Mental Health. We are seeking candidates for a Case Manager with a Bachelor's degree and minimum of 1 year experience working with individuals with developmental disabilities. Case Managers are responsible for linking, coordinating and monitoring all MCCMH related services and completing monthly home visits within Macomb County.
Responsibilities and Duties
Case Manager will be responsible for the collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet an individual’s needs. Case Managers will use communication and available resources to promote quality, effective, goal–oriented outcomes of the population served which may have differing disabilities and needs. The person in this position must demonstrate the ability to work with a wide range of diverse populations and must be sensitive to their different cultural and socio-economic backgrounds. A successful service coordination results in community opportunities and increased independence of consumers. These services will integrate the core values and mission of JOAK Macomb organization.
Note: Some parts of the job may be completed via telehealth or remotely until TBD
Qualifications and Skills
Case Manager must possess at least a Bachelor’s degree in a human services field and one year of experience with Developmentally Disabled population.
Case Manager must also possess a current, valid driver’s license/registration/insurance.
If licensed by the state at the time of hire, the Case Manager must meet all conditions and training required by law to maintain license.
Case Manager must also sign the necessary authorizations for a personal background, driving record and reference checks.

Salaried position: $37,000-$39,0000; negotiable upon experience.
