Program Assistant – Conexio Care ACT

Today, Conexio Care serves as one of Delaware's largest nonprofits, collaborating with government, community, corporate, and other philanthropic partners to maximize services for our state's most vulnerable The Program Ambassador is a member of the clinical team who assists clients and guests in gaining access to safe, timely, and quality services.

  • Description of Position: The Program Assistant is responsible for collaborating with the Team Lead to facilitate the day's operations in a supportive manner to both the team and clients. S/he maintains a professional, friendly, and welcoming environment in the office for clients and staff. S/he provides direct support to staff, including monitoring and coordinating daily team schedules and supporting staff both in the office and field around clients' needs. S/he serves as a liaison between clients and staff to include centralized, officebased communication and coordination across ACT staff and clients to promote continuity of care. S/he actively participates in the Daily Team meeting to contribute to reporting and recording of the recent ACT staff contacts with clients and their natural supports.

S/he is responsible for maintaining program files including client records, office records, and other documents for filling and storage. S/he handles matters confidential in nature and must maintain confidentiality of all clinical, financial, personnel and proprietary information.

  • Principle Duties and Responsibilities:
    • Greets and welcomes clients and guests who are visiting the program office
    • Answers office phone and routes calls to the proper personnel, takes messages and returns calls for follow-up responsesØ Attends all Daily Team meetings as well as attends other staff meetings as appropriate
    • Assists staff members with various client appointments and case management tasks, such as scheduling clients' medical and housing appointments and working with landlords
    • Assists staff members in the field, such as rescheduling another staff to see a client who is absent during contact, or looking up addresses for clients' doctor's appointments
    • Responds to walk-in visits from clients and contacts staff members when needed to assist with response to walk-ins, including medication refills, disbursement of client allowance checks
    • Handles calls from clients' family members and natural supports and other service providers
    • Maintains program CENSUS
    • Collaborates with Performance Improvement and Clinical Supervisor for tracking completion of client intakes and Consumer Reporting Forms and submits documents to Director of Program Support. Program Assistant is required to run a billing from

Carelogic on the 15th and 25th of each month

      • CAT 1- Psychiatrist, Nurse Practitioner (Prescriber)
      • CAT 2- RN/Nurses, MLC/ Master Level Clinician
      • CAT 3- Case Manager, Substance Use Specialist, Housing Specialist, Vocational Specialist and Peer Specialist
    • Tracks all client contacts each month and submits to Clinical Supervisor and Director of ACT Services for monthly billingØ Compiles client financial records including allowance check stubs and receipts of purchases
    • Verifies Medicaid eligibility for each client
    • Assists with clerical office tasks such as typing, scanning, faxing, copying and filing of documentsØ Calls in orders for work requests
    • Hand out Checks Recerts for Promise Peer
    • Completes vehicle safety inspections and maintains vehicle service records
    • Inventories and maintains adequate office supplies
    • Sends proper documents to Finance Department
    • Obtaining vitals for Highmark when requested
    • Uploading client's files into Carelogic when given Certification from ProviderØ Updates AmeriHealth
    • Hands out checks, collects receipts, utilities bills, etc., email to Finance and/or Housing
    • Maintains on call phone during the day until staff-on-call is available and deliver next morning to accurate staff
  • NETWORK FUNCTIONS:
    • Maintain and abides by OSHA requirements and guidelines
    • Observes all Fire and Safety Policies and Procedures
    • Observes all Personnel Policies and Procedures
    • Observes all Residential Policies and Procedures
    • Observes all governmental and licensing regulations
    • Observes all HIPAA Policies and Procedures and maintains confidentiality
    • Completes and returns all paperwork distributed by the Human Resource Department
    • Attends all required and scheduled trainings and meetings
    • Always maintain a professional demeanor and exercise good professional judgment in all areas of employment duties
  • EQUIPMENT/MACHINES:
    • VEHICLE REQUIREMENTS:
      • Current driver's license, driver registration and a functional vehicle Current vehicle insurance
    • EQUIPMENT OPERATIONS:
      • Telephone, cellular telephone, copier, fax, computer, calculator
    • COMPUTER SOFTWARE APPLICATIONS:
      • Has knowledge of Microsoft office programs
  • PHYSICAL DEMANDS:
    • Individual must be able to meet physical requirements of the job:
      • Sitting at a meeting table or desk, reaching, bending, standing, stooping, twisting, climbing steps, driving a vehicle, operate adaptive equipment, operate home appliances, ability to lift, push, and/or pull up to thirty (30) pounds

    • One year of experience in human services or related field
    • Knowledge of and ability to work with developmentally disabled persons
    • Excellent communication skills and organizational skills
    • Detail oriented
