Registered Nurse Case Manager
Overview:
We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive and coordinated care to patients, ensuring their needs are met and their healthcare goals are achieved. Duties:
- Conduct diagnostic evaluations and assessments to determine patient needs
- Develop and implement individualized care plans for patients
- Coordinate and collaborate with healthcare professionals to ensure continuity of care
- Monitor patient progress and adjust care plans as necessary
- Provide education and support to patients and their families
- Maintain accurate and up-to-date medical records
- Adhere to HIPAA regulations and maintain patient confidentiality
- Collaborate with managed care organizations to optimize patient outcomes
- Advocate for patients' rights and ensure they receive appropriate care
- Collaborate with interdisciplinary teams to develop discharge plans
Qualifications:
- Valid Registered Nurse (RN) license in the state of Mississippi
- Minimum 2 years of experience in acute care or hospital setting
- Experience in behavioral health is preferred
- Strong knowledge of medical terminology, procedures, and diagnoses
- Excellent communication and interpersonal skills
- Ability to work independently as well as part of a team
- Proficient in using electronic medical record systems
