Job Summary:

SPECIFICATIONS

1. PURPOSE AND SCOPE: The West Virginia Purchasing Division is soliciting bids on

behalf of the WV Veterans Nursing Facility located at 1 Freedom Way, Clarksburg, WV

26301 to establish a contract for Registered Dietician Consulting Services.

3. QUALIFICATIONS: Vendor, or Vendor’s staff if requirements are inherently limited

to individuals rather than corporate entities, shall have the following minimum

qualifications:

3.1 Vendor, or Vendor’s staff, shall have at least two (2) years’ experience as a

Registered Dietician. Proof of this experience shall be furnished upon request.

3.2 Vendor or Vendor’s employee(s) must be a Licensed Dietitian with the West

Virginia Board of Licensed Dietitians and/or other state Board of Dietitians.

3.3 Vendor or Vendor’s employee(s) must be an accredited Registered Dietitian or

Registered Dietitian Nutritionist by the Commission on Dietetic Registration.

4. MANDATORY REQUIREMENTS:

4.1 Mandatory Contract Services Requirements and Deliverables: Contract

Services must meet or exceed the mandatory requirements listed below.

4.1.1 Prior to placement in the Facility, Vendor shall perform, at vendor’s cost,

for the Dietitian:

4.1.1.1 Background check through WV Cares

4.1.1.2 Twelve-panel drug screening

4.1.2 The following information must be submitted to the Agency before the

Dietitian reports to work: Completed application or resume, at least three

(3) job references as proof of experience, copies of licenses and

registrations, negative drug screening, and WV Cares background check.

4.1.3 Agency reserves the right to refuse any staff member and/or to request

removal and replacement.

4.1.4 Successful vendor must incorporate all anticipated pre-employment

expenditures, administrative and overhead costs, taxes, FICA/FUTA,

travel expenses, incentives and any other cost incurred by vendor into their

all-inclusive hourly rate for each position and shift. Agency will not pay

any fee separately from the hourly rate.

4.1.5 Vendor must review and evaluate all new admissions no later than 72

hours after admission per regulations.

4.1.6 Vendor must be available for consultations at the request of the Agency.

4.1.7 Vendor must monitor and evaluate nutritional assessment needs and

internal feedings calculations of all tube feeding residents at least monthly

and document all recommendations/changes in resident’s charts.

4.1.8 Vendor must document all findings/recommendations in resident’s charts.

4.1.9 Vendor must monitor charts of all diabetic residents and make

recommendations and/or changes as needed.

4.1.10 Vendor must monitor charts of all residents with decubitus ulcers and

make recommendations and/or changes as needed.

4.1.11 Vendor must review nutritional assessments on facility residents and make

recommendations and/or changes as needed.

4.1.12 Vendor must be available upon request in the event of any State or Federal

Survey issues that may arise.

4.1.13 Vendor must make recommendations to the facility Physician(s) for diet

changes based on lab values, and document in resident’s chart.

4.1.14 Vendor must review diet orders for appropriateness and make necessary

recommendations.

4.1.15 Vendor must be available for consultation with the Certified Dietary

Manager as needed to discuss treatment and weight management plans.

4.1.16 Vendor must provide in-services (training) at least three (3) times per year

for Dietary and Activities staff, and nursing staff when appropriate, and

provide documentation of same.

4.1.17 Vendor must review competencies with Dietary and Activities staff, and

nursing staff as needed, and provide documentation of same.

4.1.18 Vendor must oversee and approve menus and recipes and give prior

written consent when deviations occur.

4.1.19 Vendor must work on-site at the Facility during regular working hours

Monday-Friday 8a-4p for at least two eight (8) hour days per week

Job Types: Full-time, Part-time

Pay: $32.39 - $39.01 per hour
