Lincoln County Partnership For Child Well Being, Inc. is Hiring a Family Support Coordinator Near Chandler, OK

The Family Support Coordinator is a valuable entity of the Coordinated School Health Team. This person must possess the ability to multitask and be a strong advocate for the students, families, schools and communities. The Family Support Coordinator is an advocate for the student’s environmental, physical and mental health. As the case manager, you will strive to identify needs, make appropriate referrals and provide follow-up as needed.

REPORTS TO: Program Director, Executive Director.

DUTIES AND RESPONSIBILITIES:

1. Strong verbal and written communication skills to work with teachers, administration, students, and families.

2. Receive referral information from various sources (school health team, teachers, parents, individuals, self referral).

3. Collaborate with the Coordinated School Health Team in developing individual plan of care for each student ensuring confidentiality and HIPPA compliance throughout the process.

4. Research resources needed and advocate for the student’s needs to make sure they receive the right care for each situation.

5. Assist with securing services needed (i.e.; Sooner Care, SNAP, Free & Reduced lunches, Medicaid etc.

6. Be a critical thinker and problem solver to determine the best plan of care for each individual case. Follow each case’s progress and make adjustments as needed to improve outcomes. Establish and conduct regular contact with the individual, families and services until needs are met.

7. Flexibility to change plans if not getting the best results for the student.

8. Organize and manage several different cases at once.

9. Safeguard and maintain individual records to keep files complete, confidential and in a secure area. Obtain and document “signed release of information agreementprior to sharing private information to others.

10. Working with the school to establish student and family support programs or services. (Resource for clothing, backpack program, resources, donations etc.)

11. Provide information and referral services to the students and families that seek out assistance.

12. Maintain an updated list of each community’s resources.

13. Organize, attend and facilitate each Coordinated School Health Team staffing. Update the current caseload, identify additional coordinated services needed per case or discuss new referrals.

14. Get to know the school’s administration, staff, environment and climate. Lead or assist in school health programs as needed or assigned by Program Director.

15. Promote the investment of the school health programs by building relationships in each school and communities.

MINIMUM QUALIFICATIONS

1. Must have an interest and investment in working with children, families with multiple areas of concern and backgrounds.

2. Bachelor’s Degree or Higher from an accredited university.

3. Experience in social services, health or an educational school setting is useful.

4. Positive attitude, exhibits the ability to problem solve and have the willingness to communicate a problem or issue of concern to appropriate individuals before it becomes a major barrier.

5. A commitment to abide by the Partnerships’ vision and values.

6. Must have good time management skills.

7. Participate in positive health related activities.

8. Ensures adherence to confidentially, HIPPA requirements and social media discretion.

9. Assists in data collection as needed.

10. Any tasks or needs identified by the Program or Executive Director.

11. Possess a valid driver’s license and reliable transportation.

12. Must consent to a background check.

13. Provide a current resume’ with a minimum of 3 references.

Job Type: Full-time

Pay: $62,500.00 per year
