Position Purpose:

Advocate for designated clients. Participate as a team member in the delivery of addictions treatment services to clients and their families and facilitate the issues of recovery into their daily living situations. Provide continuity of program services in an ethical, legal and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Promote the treatment concepts and work cooperatively with all staff to implement treatment services.

Job Responsibilities:

  • Provide toxicology testing, case management, and transportation services to outpatient and intensive outpatient services clients
  • Provide urine screens and breathalyzer testing to clients to monitor sobriety during recovery
  • Report and document significant findings of each client as they relate to development and progress or lack of progress towards individualized treatment goals.
  • Accompany clients as an advocate to places such as court, medical clinic, public assistance office or other mental health services as recommended.
  • Remain active as advocate and when possible, is involved in case management planning when client is transferred to another level of care such as inpatient chemical dependency or psychiatric services.
  • Provide referral and assistance for client’s immediate needs such as housing, food or other risks of potential harm.
  • Work closely with other outreach worker to share duties as needed and communicate special circumstances of each client when appropriate to insure consistency of care in times of absence.
  • Complete documentation according to the Agency's policies and procedures manual concerning client service record and other reporting as directed.
  • Serve as a member of the outreach Team and participate in all team meetings and activities.
  • Understand and comply with all of the principles established by the Rosecrance Corporate Compliance Program and Code of Conduct.

Position Requirements:

  • Bachelor's degree in criminology/criminal justice, psychology, sociology, social work, or equivalent
  • Must have valid driver's license and insurance
  • Must be willing and able to operate agency vans to transport clients
  • Experience working with diverse people is a plus

Pay: Based on education, experience, and credentials

Schedule:

  • Monday-Friday hours will be between 9am-730pm (on a rotation basis)
