This will be a Part-Time day shift position

Position Summary :

Provides Personal Care Services to clients when necessary to maintain their optimum level of functioning assisting with activities of daily living within Graceworks at Home / Health at Home services.

Continually works to promote Client independence

Job duties :

• Provides scheduled personal care needs of the highest quality to clients in their living environment under the Graceworks at Home / Health at Home Care Program.
• Under the direction of a registered nurse or therapist, provides care in response to the outlined care plan in the home.

This includes; hands on care, assisting with medications reminders that are ordinarily self-administered, assistance in ambulation or exercise, and the performance of simple procedures as an extension of therapy or nursing services, as permitted by State of Ohio practice acts.

• Attempt to meet the physical, emotional and spiritual needs of clients.
• Complete paperwork needed to support services and billing requirements.
• Attend scheduled in-services, training hours and staff meetings.
• Responsible for promptly reporting any changes in client’s condition (such as skin breakdown, weigh changes, etc), incidents, theft, damage, and / or abuse to the Administrator of Health at Home, LLC
• Adhere to all policies and procedures of the organization and all federal and state laws.
• Graceworks at Home / Health at Home office as needed.

Qualifications : Due to our federal funding, we are a drug free workplace (including marijuana).

A pre-employment drug screen is required for all positions.

Valid driver’s license and safe driving record

One year of verifiable nurse assistant experience in a supervised setting.
