**Licensed Practical Nurse (LPN/LVN) -

**Position Overview:**

• Licensed Practical Nurse (LPN/LVN) responsible for skilled nursing care under RN supervision.
• Delivery and coordination of quality patient care in compliance with physician orders.
• Observes and assesses patient condition, reporting changes to the supervisor or physician.
• Timely documentation of all activities, assessments, and coordination of care.

**Essential Job Functions:**

• Participate, implement, and update nursing care plans.
• Takes appropriate nursing action based on assessments.
• Recognizes and intervenes in changes in patient needs.
• Ensures infection control measures are followed.
• Educates patients and families on disease process, self-care, and prevention strategies.
• Maintains knowledge through educational programs and skill evaluations.

**Requirements:**

• Graduate of an accredited school of nursing.
• Current, unrestricted state license as a Licensed Nurse.
• Current CPR certification.
• Demonstrated proficiency in clinical assessments, documentation, and compliance.

**Preferences:**

• Six (6) months of recent experience in a clinical care setting.
• Home health experience.

**Physical Requirements:**

• Must be able to travel, lift 50 pounds, and reposition patients and equipment.
• Various physical activities, including walking, standing, bending, and climbing stairs.
