This is a full-time on-site role for a Case Manager at Phoenix Rising Behavioral Healthcare. As a Case Manager, you will be responsible for providing comprehensive case management services to clients. This includes conducting assessments, developing individualized treatment plans, coordinating services, advocating for clients, and monitoring progress. You will work closely with a multidisciplinary team to ensure that clients receive the support and services they need. Qualifications

• Strong organizational and time management skills
• Excellent communication and interpersonal skills
• Ability to work effectively in a team and collaborate with other professionals
• Knowledge of behavioral health diagnoses and treatment modalities
• Familiarity with community resources and referral processes
• Experience in case management or related field
• Bachelor's degree in social work, psychology, or related field (Master's degree preferred)
• Valid driver's license and reliable transportation
• Bilingual in English and Spanish (preferred, but not required)
