Job Description

The Independent Living Supervisor is part of the administration team for the entire Adult Services Division and is the direct supervisor of the In-Home Case Coordinator team. This combination position is expected to devote approximately 50% of their time to the administrative tasks of supervisor and team lead, 25% of their time to a caseload of their own and 25% of their time to intake and assessments for new clientele. The right candidate will be enthusiastic about team-building and making connections with a mobile workforce and will enjoy being on the road as much as they enjoy their office and administration roles.

The Case Coordinator teams needs a leader with strong resource social worker abilities; the right candidate for this position knows available local, state and regional benefits and aide and can guide the team to access these. This position recruits, onboards, trains and assigns Case Coordinators as needed, conducts site and home visits with their Case Coordinators for the purposes of monitoring progress and eliciting consumer feedback and provides regular direct supervision to Case Coordinators and acts as coverage in the absence of members of the Case Coordinator team.

Empower Case Coordinators act as paid surrogates for consumer PCA programs. The best candidate for this position has or will develop an understanding of the Personal Care Assistant program and the intersection of the PCA program with other Empower provided services.

We are most excited about a candidate who demonstrates strong writing skills and the ability to assess bio-psycho-social factors, plan interventions and report those in narrative form for intakes and assessments.

Qualifications

• Must possess a graduate degree in human services: mental health, social work or related.
• Preference will be given to those candidates with independent licensure.
• Supervisory experience preferred.
• Familiarity with disability population or related population.
• Familiarity with ethical and other considerations in the provision of services in the homes of individuals.
• Computer competency, particularly with MS Word, Excel and Outlook.
• Ability to work independently and as part of a team.
• Able to travel independently throughout Massachusetts

Pay: $60,000.00 - $75,000.00 per year
