Job Summary and Qualifications

The Social Work Case Manager is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.

What you will do in this role:

• As the Social Worker, you will assess the patient’s psychosocial status, to confirm probably post-acute needs, and to detect any potential barriers to a successful transition in to the community.
• You will collaborate with the case manager, the physician and family to determine the best course of action.
• You will demonstrate knowledge of human growth and development, assessment, range of treatment appropriate to the ages of patients served.
• You will screen all patients on assigned unit/service line by covering the selected number of key screening indicators that consistently signal the need for a more in-depth transition plan evaluation by the social worker.
• You will respond to all consults in a timely and efficient manner.
• You will collaborate with case manager, nursing staff, ancillary departments and patient/family in formulation of the transition plan.

What qualifications you will need:

• Master’s Degree in Social Work required
• 1-3 years experience in an acute care medical setting

Parkridge Medical Center is a 270 bed hospital. We are near downtown Chattanooga. We feature comprehensive diagnostic, medical, and surgical services. These services include cardiovascular surgery and cancer treatment. These services include intensive care, orthopedic and spine surgery, and rehabilitation. The facility includes a state of the art neurosurgery program. Patients also have access to OB-GYN services. Patients have access to a level III Neonatal Intensive Care Unit. They also have access to (NICU) and behavioral health treatment centers for children and adults.
