For a male 42 yrs. old

He is a Quadriplegic, trach, vent dependent, foley catheter.

You will need the following documentation that is required:

* RN/LPN License *Level 2 AHCA Background Screening

*Health Statement/Physical (in the last 6 months and not be completed by a Chiropractor)

*TB Test ( in the last year)

*CPR Certification (must be in-class certification- American Heart Association, or Red Cross Accredited)

*CEU's :

*Alzheimer's Disease Initiative CEU

* Assistance with Self-Administration of Medications CEU

* Communication with Cognitive Impaired Clients CEU

* HIV CEU

* Infection Control CEU

* Reporting Elder Abuse/Neglect/Exploitation CEU

* Resident Rights CEU

* Valid Driver's License

* Proof of Auto Insurance

* Social Security Card

* Resume

* Reliable Transportation

* Working Smart Phone
