Homeward Bound Nurses are responsible for safely and professionally providing exceptional nursing care and life enrichment services to the people we serve. Nurses are also responsible for leading others through training, coaching and mentoring frontline Nurses and Direct Care Staff. 

Responsibilities Include

• Administer medication and treatments as ordered by a doctor. Enter new med orders into eMAR. Complete narcotic counts. Assist with inventory management.
• Assess and monitor each individual's health situation and complete thorough charting.
• Transport and accompany individuals to appointments and coordinate the visit.
• Seek to understand each individual, their goals, dreams and interests, and support them in living their best life!
• Actively support individuals in making their own choices, directing their own care as much as possible, and pursuing their interests.
• Assist individuals in selecting and planning outings, accompany the individuals on outings.
• Actively engage in conversation and activities with individuals at all times.
• Encourage individuals to participate to the fullest extent possible in completing activities of daily living.
• Work safely, performing job tasks as trained and asking for proper instruction when in doubt. Abide by safety rules.
• Provide initial and ongoing training to employees as assigned.

Qualifications

1. Active LPN or RN licensure in the state of MN. 
2. Active First Aid and CPR certification. 
3. Meet the licensing, contractual, and other mandated requirements of the state and county. 
4. Valid Minnesota Driver’s License with acceptable record required.
5. MN Department of Human Services Background Clearance.
