Excelsior Care Group is Hiring a Part-Time Social Worker Near Briarcliff Manor, NY
Salary: $28-$33 per hour
Responsibilities:
- Conduct interviews with residents and families to gather social history.
- Engage residents and families in planning social service programs when possible.
- Provide solutions for social and practical environmental issues, including seeking financial assistance, discharge planning, and making referrals to other community agencies for specialized assistance.
- Coordinate with administration, medical and nursing staff, and other departments to plan social service programs and activities.
- Participate in facility surveys (inspections) conducted by authorized government agencies as necessary.
- Communicate residents' social, psychological, and emotional needs to the medical staff, attending physician, and other care team members.
- Bachelor's degree in Psychology required
- Minimum of 1-year experience working in a Skilled Nursing Facility
