Putnam Ridge is a 160 bed long-term care and rehabilitative facility located in Brewster, NY. We pride ourselves on providing superior quality and service to our residents and patients.
We are actively seeking an experienced full time Registered Dietician. Knowledge of resident care plans, Meal Tracker and Sigma Care a plus.
Key Duties
•Conducts a nutrition assessment for residents
•Develops and implements an individualized plan for nutrition intervention based on nutrition assessment and evidence-based standards
•Collaborate with the interdisciplinary team
•Experience with electronic charting
•Works cooperatively with food service director to ensure conformance to diet prescriptions
Required Licenses/Certifications
•Dietetic Registration
