Clara Martin Center is Hiring a Case Manager Near Bradford, VT
Seeking a fun, energetic and caring full time human services professional to provide case management, outreach and referral services to adults with mental health and/or substance abuse related issues. Services are provided on an outpatient basis in the office and in the community.
Case Managers are part of a team of professionals including Therapists, nursing, vocational and support workers. We have positions available in our Bradford and Randolph offices.
Essential functions include:
- Providing case management, care coordination, supportive counseling & problem solving to adults, groups and family members.
- Documenting clinical services in an electronic medical record.
- Gathering & completing paperwork to open charts, document progress and discharge when appropriate.
- Providing outreach & support to clients in the community.
A BA degree in human services, psychology or related field strongly preferred.
