Are you a compassionate and dedicated Licensed Social Worker looking to make a meaningful impact in the lives of patients in their homes? We are seeking a qualified individual to join our home health care team, providing vital mental health services to individuals in their home.
Key Responsibilities:
- Assess patients' mental health needs.
- Provide support services to patients and their families.
- Maintain accurate and timely clinical documentation.
Position Qualifications:
- Social Work license.
- Reliable transportation and valid auto insurance.
- Strong clinical abilities, effective communication, dedication to quality patient care.
- Prior home health experience preferred; training available for suitable candidates.
Hours: Monday – Friday, flexible hours on a PRN basis.
Work Type: In-person / traveling within a reasonable distance to patients' homes.
Pay: $40 - $45 per hour.
Job Type: PRN
Pay: $40.00 - $45.00 per hour
Expected hours: 1 – 30 per week
