The Licensed Practical Nurse (LPN) is a licensed nurse who participates in the plan of care to provide competent, compassionate and individualized nursing care to patients and their families - as developed by a registered nurse or nurse practitioner.The LPN also participates in the medical plan of care under the supervision of the physician or nurse practitioner and in activities to facilitate safe and efficient practice operations.
Participates in the plan of nursing care as developed by and under the supervision of a registered nurse or nurse practitioner.
Communicates the plan of care and other pertinent information to/from other team members.
Provides nursing care appropriate to age group of patients consistent with the practice scope of service.
Observes, reports, and documents pertinent patient information, in accordance with the standards of documentation for ambulatory care nursing.
Performs nursing procedures within the scope of practice of an LPN and as outlined in the Nursing Procedure manual.
Collects data to help the nurse practitioner evaluate outcomes of nursing care for patients and modify the care plan as appropriate.
Education of patients and families regarding proper use of common medications and possible side effects.
Works with the nurse practitioner to define standards of excellence for patient care.
Participates in and contributes to performance improvement activities.
Assures efficient patient flow through the office.
Responds to needs to expedite visits and keep on schedule.
Participates in the medical plan of care under the direction of the physician or nurse practitioner.
Assists provider with patient treatments within the parameters defined by the Department of Nursing.
Administers medications within the parameters defined by the Department of Nursing after successful orientation.
Receives patient messages, reviews with provider, and acts as directed.
Faxes, mails or telephones in prescription renewals as directed by provider.
Assists provider in coordinating care with/making referrals to community agencies.
Participates in orientation of new permanent and temporary staff members.
Teaches and directs other clinical support staff for whom she/he is responsible.
Appropriately utilizes MGH nursing resources.
Participates in daily activities conducive to the comfort and safety of patients, their families, and practice staff.
Appropriately utilizes equipment and supplies.
Provides input to supervisor regarding practice needs.
Participates in practice and other committees for formulation of practice policies & procedures.
Professional development - maintains and updates clinical knowledge and skills.
Collaborates with nurse practitioner to identify/meet own learning needs.
Participates in relevant continuing education programs.
Adheres to regulatory requirements for annual training.
