The Occupational Therapist (OT) specializes in improving residents' daily living skills and independence in our skilled nursing facility.
Responsibilities:
- Assessment and Evaluation: Perform comprehensive assessments of residents' functional abilities, with a focus on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Individualized Care Plans: Develop tailored intervention plans to improve residents' independence in daily activities.
- Section GG Data Collection: Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, Significant Change assessments, BIMS, PHQ 2-9, and other clinical data as needed.
- Treatment: Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modification.
- Education: Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check-in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care.
- Documentation: Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs(InnovaHealth/ Net Health/PCC)
- Interdisciplinary Collaboration: Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, for holistic resident care.
- State licensure as an Occupational Therapist
- EMR and MS office applications.
- CPR/BLS certification.
- Knowledge of Section GG data collection and documentation requirements.
- Strong clinical and interpersonal skills.
- Excellent written and verbal communication skills.
- Ability to supervise assistants, technicians/aids and student/interns.
- Modality Certification
- Experience in LTPAC/LTC clinical programs are always a plus
