Council on Aging of Southwestern Ohio is Hiring an Intake (ADRC) Specialist (Part Time) Near Blue, OH

10:00 am to 2:00 pm Monday-Friday
Council on Aging of Southwestern Ohio (COA) is a nonprofit organization dedicated to enhancing quality of life for older adults, people with disabilities, their families and caregivers. COA is a state-designated Area Agency on Aging, serving a 5-county region. One call to COA links people to the wide variety of agencies, information and programs that serve older adults and people with disabilities.
COA has more than 300 employees, close to 300 contracted providers, and an annual budget of $115 million.

Position Description:
Responsible for telephone screening and/or assessment of individuals to determine program eligibility for appropriate long term care options or facilities, as well as provide information and resources.
Essential Job Functions:
  • Assist, educate, and support callers from the COA 5 county area who are attempting to negotiate the aging and disability network or coping with difficult long-term care decisions, about available community resources and services
  • Complete telephone and computerized screening from all referral sources, to determine an individual’s appropriateness for COA administered services and complete referrals to COA and community agencies as appropriate
  • Assist callers with crisis situations and follow any COA protocols as needed.
  • Work closely with COA staff on communicating consumer related issues as determined through conversation with consumers or referral sources.
  • Maintain current knowledge on all COA programs, Medicare, Medicaid, long term care topics and other issues in order to provide assistance to callers and/or their care givers.
  • Adhere to COA confidentiality policy, consumer rights and responsibilities, and HIPPA protocol as related to all individual information and interactions.
  • Assist consumers in selecting the best Medicare D options by providing information and education.
  • Assist as needed to provide training or education to the community or staff on aging and disability resources and issues
  • Obtain training and maintain certification for OSHIIP, AIRS, or other programs, as recommended by the Manager.
  • Maximize use of technology and reporting for continued enhancement/improvement of operations
Qualification Requirements:
  • Bachelor’s degree in social service field preferred. Two years of work experience in relevant field may serve as an equivalent substitute for each year of college.
  • AIRS Certified or able to obtain AIRS certification within one year of employment.
  • Have at least one year prior experience in health care, community outreach, community resources, medical social work, geriatrics, or providing individuals with information about options available to meet long-term care needs or with related information such as that which is provided through information and referral or assistance.
  • Good working knowledge and understanding of issues related to the elderly and disabled population.
  • Tolerant, empathetic, good interviewing capability.
  • Knowledge of community resources within Southwestern Ohio.
  • Excellent oral and written communication skills.
  • Ability to adapt to frequent change with composure.
  • Builds cooperation among team members; treats team members equitably.
  • A valid driver’s license and reliable transportation.
  • Excellent computer skills, including the Windows operating system, MS Word, MS Excel, data entry and the ability to learn new software applications as needed.
  • Ability to travel within the state of Ohio for training.
  • Respect for the cultural, spiritual, racial and ethnic beliefs and/or lifestyle choices of others; supports and manages diversity inside the team and COA.
