Home care experience preferred by not required. We are looking for highly professional, self motivated LPNs and RNs who love working with the adult and geriatric homebound population. Qualifications

• Successful completion of nursing program accredited by National League of Nursing preferred.
• HCHB experience preferred
• Licensed to Practice professional nursing in the state
• Minimum of two years of nursing experience preferred; 1 year licensed RN experience required.
• Previous Home Health experience preferred
• Ability to travel during business hours

Responsibilities

• Provides initial and ongoing comprehensive assessments of client's needs and other services requiring substantial and specialized nursing skill.
• Coordinates the total Plan of Care and maintains continuity of client care by liaising with all medical professionals involved in client's care.
• Timely responses to inquiries and concerns from patients and their caregivers/guardians.
• Develops, prepares, and maintains accurate and timely individualized client care progress records.

Your only weekend responsibility is a one-weekend day per month rotating schedule where you cover all planned or PRN patient visits, as scheduled. You know the weekend day you are responsible to work, and you are provided two days off during the adjacent weeks, (i.e. Thursday for Saturday and Tuesday for Sunday), to ensure you have the time you need to address your personal and family responsibilities.
