Job Location: Our Social Workers see patients in the most ideal setting - the patient's home!
Our office is located in Berwick, PA
Shift(s): Per diem - Office hours Mon - Fri 8:30 - 5:00.
Duties and Responsibilities include, but not limited to the following:
- Assess patient care and plans with patient, family, and physician.
- Assist the patient with ADL and facilitate their efforts towards self-sufficiency and independence.
- Assess home environment regarding self-care, coping, and living arrangements.
- Communicates effectively with patient, patient’s family, agency staff, physician, and other involved disciplines (social services, area on aging, meals on wheels etc.) to carry out treatment plan objectives.
- Master's Degree in Social Work
- Current Pennsylvania LSW / LCSW preferred
- Must have a current driver's license
- Ability to travel to case assignments as assigned
- Ability to work independently with minimal supervision and manage time effectively
- Strong interpersonal skills
