Company Summary

A fast-growing Home Care agency, is seeking a Registered Nurse (RN) to provide initial and follow-up homecare assessments in Berlin, MD. Will need reliable transportation. Homecare experience preferred. Compensation terms are flexible for the right candidate, either Per Diem or Part-time. Flexible hours but prefer individual who is available on mornings.

Position Summary

• To ensure quality and safe delivery of home health care services.
• To coordinate provision of home health care services that reflect agency philosophy and standards.
• To ensure quality and safe delivery of nursing care services delivered within the guidelines of the ANA Standards of Care and the policies of company

Responsibilities

Patient Care

• Conducts initial patient assessments and reassessments and plans patient care per physicians plan of care
• Responsible for discharge planning and the discharge process..
• Implements current Nursing Practice.
• Consistently adheres to universal precautions, aseptic technique and infection control guidelines

Professionalism

• Ensures quality and safe delivery of home health care services.
• Consistently demonstrates professionalism through appearance, performance and communication.
• Demonstrates respect for the opinion of others.
• Consistently assumes and follows through on the responsibility for assignment.

• Consistently submits time card and patient records completed in appropriate timely manner.

• Assumes responsibility for reading and comprehending all posted notices, communications and policies/procedures.

Leadership & Teamwork

• Oversees staff assignments and staffing patterns.
• Provides guidance to assign appropriate aides and skilled personnel to a client to meet the clients needs.
• Supervises and directs field personnel in providing care.
• Coordinates with community agencies to provide for continuity of care.
• Performs additional duties as assigned by DCS or President.

Communication

• Demonstrates the ability to communicate effectively with the client and his/her family members.
• Interprets to the client and family implications of the client's health status.
• Demonstrates the ability to communicate effectively with other members of the health care team and staff of the agency.
