Pathways operates a Medicare REACH ACO, a Medicare Advantage MSO, and a Commercial MSO responsible for approximately 20,000 patients throughout North-West/Central Florida. Our service area is The Villages, down to St. Petersburg, across to Mount Dora, and areas in between. Pathways also operates a robust hospital medicine group, manages several of its affiliated medical practices, and an insurance agency.

Job Summary - Belleview, FL location - Bilingual Preferred

Pathways Health Partners is seeking a skilled, experienced, and patient-centric APRN to join our team. In this position, you will diagnose, treat, and counsel patients on their current medical conditions and educate them on preventative health measures, including performing annual physicals. Demonstrating compassion and genuine kindness with all patients.

Role and Responsibilities

  • Meet with patients to assess their symptoms; inquire into the nature of their illness or injury
  • Accurately identify and diagnose medical conditions
  • Thoroughly examine patients and attend to their chief complaint
  • Answer and advise on any other questions or concern the patient might have
  • Assign appropriate treatment plan according to each patient’s needs
  • Educate and inform patient of necessary treatment plan and any follow-up appointments
  • Perform clinical tests to better inform diagnosis
  • Perform in-office treatment if possible
  • Look for anything unusual or abnormal
  • Prescribe appropriate medicines or therapy
  • Calculate correct medication dosage for each patient and educate on proper use
  • Order outside diagnostic tests and lab work as needed (blood/urine test, tissue samples, imagery, etc.)
  • Analyze and share results of diagnostic testing with patient
  • Continually chart and maintain patient information on paper and in computer system
  • Conduct follow-ups with patients to monitor and track their condition and progress
  • Refer patient to a medical specialist when needed
  • Maintain confidentiality and always work with respect and integrity
  • Encourage preventative health measures including proper diet, rest and exercise
  • Direct, train, and supervise medical staff including nurses, and med techs on proper procedures and protocol

Qualifications and Education Requirements

  • Current and clear Florida APRN License
  • Approved Medicare with PTAN and status with Blue Cross of Florida, United Healthcare, and Cigna (must be in place to begin work)
  • Able to prescribe controlled substances, (DEA license and DCS)
