¨Home Care Companions provide service to individuals in their own homes and communities who needassistance caring for themselves as a result of old age, sickness, disability and/or other inflictions.Homecaremayincludelighthousecleaning,laundry,mealpreparation,transportation,companionship,respiteandadviceonsuch thingsasnutrition,cleanlinessandhouseholdactivities.

¨HomeCareCompanionsareresponsibleforensuringthatserviceisdeliveredinacaringandrespectfulmanner,inaccordancewithrelevantAgencypoliciesandindustrystandards.

Reporting Relationship

ReportstoSupervisor

Responsibilities/Activities:

¨Providecompanionship,friendshipandemotionalsupport.

¨Talk,listen,shareexperiences,playgames/cards,readtoclientetc.

¨Helpkeepclientsincontactwithfamily,friendsandtheoutsideworld.

¨Providetransportationtomedicalappointments,grocerystoreanderrands.

¨Accompanyclientstorecreationaland/orsocialevents.

¨Assistwithplansforvisitsandoutings.

¨Writeortypeletters/correspondence.

¨Organizeandreadmail.

¨Plantripsandoutingsandpossiblytravelwithclients.

¨Teach/performmealplanningandpreparation.

¨Performlighthousekeeping.

¨ParticipateontheCareTeambyprovidinginputandmakingsuggestions.

¨EnsureserviceisdeliveredinaccordancewithAgencypolicies,proceduresandindustrystandards.

¨Monitorsuppliesandresources.

¨Evaluatetheprogramandmakerecommendations,asindicated.

¨Followthewrittencareplan.

¨Assistinbasicclienttransfersprovidingtheclienthasbeenassessedasbeingcapableofambulatingwithout assistance; and/or, providing another trained caregiver (including family) is involved in thetransfer.

¨CarryoutdutiesasassignedbytheSupervisor.

¨Observetheclient’sfunctioningandreporttoSupervisor.

¨Completeandmaintainrecordsofdailyactivities,observations,anddirecthoursofservice.

¨Developandmaintainconstructiveandcooperativeworkingrelationshipswithothers.

¨Makedecisionsandsolveproblems.

¨Assistwithpetcare.

¨CommunicatewithSupervisorandco-workers.

¨Attend orientation, in-service training sessions and staff meetings.

RequiredKnowledge

¨Knowledgeofhomemanagementskills.

¨Knowledge of principles and processes for providing client services, including needs determinants,meetingqualitystandards andevaluation ofclientsatisfaction.

¨KnowledgeoftheEnglishlanguage.
