Preferred Care Health Centers is Hiring a Physical Therapist Near Bay, MI
Carriage House Nursing and Rehab is seeking an enthusiastic Physical Therapy (PT) to join our team. This is a full time, permanent position that provides in-house therapy. Our newly renovated facility has all new equipment, complete with ACP modality suites along with clinical support. $4000 sign on bonus!
Responsibilities
- Assess each patient and develop an appropriate treatment plan with measurable goals and time frames
- Prepare and submit clinical and progress summaries based on identified and attained goals
- Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
- Provides resident/patient treatment which meets their needs and uses current treatment knowledge
- Consistently demonstrate the Facility’s interdisciplinary approach to resident/patient care.
- Contributes to professional development of self and others.
Qualifications
- Current Physical Therapist License for State of Michigan
