POSITION: Case Manager

PROGRAM: School-Based Services

DIVISION: Children, Youth and Family Services

SUMMARY OF POSITION: Develops and /or delivers ongoing community-based assessment, treatment and supports for children and youth experiencing a severe emotional disturbance and their families. Full time with benefits.

SPECIFIC DUTIES & RESPONSIBILITIES:

  1. Responsible for supervising up to 10 Behavioral Interventionist staff.
  2. Develops and / or delivers services to assist clients in gaining access to needed medical, social, educational, vocational, housing, financial and other services.
  3. Maintain contact with other professional service providers for the purpose of case review or consultation regarding services to a specific client.
  4. Develop treatment plans to deliver or provide for the delivery of services and interventions identified by the treatment team.
  5. Assist in developing natural support networks for children and youth experiencing a severe emotional disturbance and their families.
  6. Assist children and youth experiencing a severe emotional disturbance and their families in accessing community resources.
  7. Participate in ongoing treatment and educational team meetings.
  8. Work cooperatively with schools, social rehabilitative services and other agencies providing services to children and youth experiencing a severe emotional disturbance and their families
  9. Provide and maintain ongoing electronic medical records for all clients. Complete all assigned paperwork accurately and within the identified timeframe. Maintain client records to include writing contact notes, treatment plans, treatment team summaries, and all other required paperwork relevant to providing client services.
  10. Attend staff meetings at WCMH as required.
  11. Ability to physically restrain children and youth who are a danger to self or others.
  12. Willing to learn crisis de-escalation and passive restraint techniques.
  13. Problem solve and manage crisis.
  14. Maintain on-going communication with school staff.
  15. To familiarize, implement and comply with WCMHS policies and procedures.
  16. Other duties as assigned by supervisor.

QUALIFICATIONS:

Education: Bachelor’s Degree in human service or related field required. Master’s Degree preferred.

Experience: Two years of human service delivery with children and families preferred. Experience providing direct instruction and therapeutic services to children with challenging behaviors preferred. Possession of a valid driver’s license, excellent driving record and access to a safe, reliable, insured vehicle are required. Ability to lift and carry 50 pounds and execute physical restraints.
