Overview:
We are seeking a dedicated Wound Care Nurse to join our team at Brooke Knoll Village in Avon, Indiana. The ideal candidate will have expertise in wound care management and a passion for providing high-quality patient care.
Responsibilities:
-Must conduct full body assessments of the resident upon admission/ re-admission, assessing for any red or open areas, documenting those areas appropriately and making appropriate physician and family notifications. Should no red or open areas be noted, determine potential risk for skin breakdown and implement appropriate interventions accordingly.
- Monitor twenty-four hour reports and converse with nursing staff to be alerted to any newly observed red or open areas. Upon notification, immediately assess and document these areas appropriately and ensure appropriate physician and family notification has been conducted and documented. Initiate necessary tracking forms for ongoing monitoring and documentation of applicable areas
- Reassess weekly the progress, or lack of progress, in the healing of open areas as well as obtain weekly measurements and document accurately.
- Review ordered treatments for open areas, assessing the efficacy of treatment, and making recommendations for change of treatment to those areas that seem to have no response to the current treatment.
- Assess for the proper use of equipment to prevent skin-to-skin contact, educating the nursing staff as needed.
- Prepare weekly skin assessment report to be presented to the Director of Nursing for review as well as other involved interdisciplinary team members (i.e., Director of Food Service, Dietician, Administrator, etc.).
-Notify MDS Coordinator when the severity of decubitus ulcer could warrant a significant change comprehensive assessment.
- Assure on admission that pressure reducing devices, if needed, are in place whether it be on the bed, in the wheelchair or both. Assessing also the need for heel/elbow protectors, etc.
Requirements:
- Must be a nurse in good standing and currently licensed by the State of Indiana
- Previous experience in sterile processing and wound care management
- Wound certification desirable.
- Previous Long Term Care experience desirable.
- Must possess health certificate/examination as required by State agencies
