Responsibilities
- Point based system of 6 points daily to be considered full time/ 30 points per week
- Start of care and resumption of care = 2pts
- Skilled visit = 1pt
- Over 60 points in a 2 week pay period is paid additionally as a flat rate per diem
- Mileage per day is less for RN Case Managers versus Baylor RNs
- OASIS for Medicaid (only 2% of patients)
- MatrixCare (formerly Brightree)
- Graduate of an accredited school of nursing
- Connecticut State Registered Nurse license in good standing
- BLS Certified.
- 1yr General Home Health experience preferred: no pediatrics, behavioral health, palliative or hospice care and no IV certifications needed
- One weekend every 6-8 weeks
- Must sign up to work 2 holidays per year
- Choice of Baylor RN or Case Management RN
- Health, dental and long and short term disability offered to FT employees
- Health Insurance is through Centivo
- Dental, Disability and Life Insurance is through Anthem
- 401k contribution eligibility after 90 days then after 1 year is company match
- 25 days of prorated PTO per year (includes Holiday, sick, vacation, and personal time)
- $35/month cell phone allowance (monthly bill required)
- Privately owned and clinician managed company
- Mileage is paid $0.50/mile when logged between patient's homes
- Salary $80,000-$90,000/yr, based on experience
