NorthLakes Community Clinic is Hiring a Peer Specialist, Certified Near Ashland, WI

Description

We are seeking a full time (1.0) Peer Specialist, Certified to join our Ashland Clinic team!

Hours: Monday - Friday, 8:00 am - 4:30 pm

Peer Specialists support our teams in delivering person-centered care and help fulfill our mission to respond to the needs of our communities with an integrated array of quality services.

The Peer Support Specialist partners with each person to engage and support them in mental health and/or substance use recovery, connect to resources and services, and promote wellness through sharing of lived experiences.

Duties include but are not limited to:

  • Identify as a person in mental health and/or substance use recovery; Intentionally share own recovery story as appropriate to assist peers and care teams in understanding pathways in recovery.
  • Establish supportive relationships with peers to promote recovery.
  • Assist peers in understanding the purpose of peer support and the recovery process.
  • Work with peers to facilitate change in patterns and behaviors
  • Work with peers and care teams to create an environment of respect for peers which honors individual choice.
  • Mutually establish acceptable boundaries with peers and care team members
  • Support peers in connecting to services and resources and explore options that may be beneficial to returning to emotional and physical wellness.
  • Encourage and assist peers to construct their own recovery and wellness plans, which may also include proactive crisis and/or relapse prevention plans.
  • Provide culturally-sensitive and age-appropriate services.
  • Encourage peers to focus on their strengths, natural supports, develop their own recovery goals, and strengthen their valued roles within the community
  • Working knowledge and relationships with local service providers and resources.
  • Work closely with care team to promote comprehensive and coordinated care.
  • Act as a liaison between the person/family and service agencies (i.e. schools, Department Human Services, hospitals, support groups, etc.).
  • Manage assigned caseload of participants in collaboration with care team
  • Attend regular staff meetings, briefings, huddles, trainings and other meetings as requested.
  • Work with program manager to collect program data and relay information.
  • Record patient care in the EHR and other software.
  • Understand and demonstrate knowledge of HIPAA.
  • Work with manager to identify community involvement and outreach needs.
  • Collaborate on efforts related to organizational change to promote trauma informed care and recovery orientation.
  • Collaborate on efforts to develop a peer specialist program to support individuals in recovery.
  • Participate in trainings and meetings as assigned.
  • Other duties as assigned.

Requirements

Qualifications:

  • A High School diploma or equivalent is required.
  • Completed Peer Specialist Certification training and have passed certification and/or training as a Recovery Coach with CCAR Recovery Coach Academy or equivalent and willingness to become certified as a Peer Specialist within 12 months of employment.
  • Identify as a person in mental health and/or substance use recovery.

Preferred Skills:

  • Strong communication skills.
  • Ability to problem-solve and manage competing priorities.
  • Excellent computer skills and experience using Electronic Health Records.
