Registered Nurse (RN) - Home Care

Overview:

We are seeking a compassionate and skilled Registered Nurse (RN) to join our team in providing high-quality home care services to patients in Florida. As an RN in home care, you will play a crucial role in delivering personalized care to patients in the comfort of their homes. You will assess patient needs, develop care plans, administer treatments, and provide education and support to patients and their families.

Responsibilities:

Conduct comprehensive assessments of patients' health status.

Develop individualized care plans in collaboration with the patient, their family, and other healthcare professionals.

Perform skilled nursing tasks, such as wound care, medication management, IV therapy, and catheter care, in accordance with physician orders and nursing standards.

Monitor patients' vital signs, symptoms, and reactions to treatments, and communicate any changes to the healthcare team.

Provide patient and family education on disease management, medication administration, lifestyle modifications, and other relevant topics.

Coordinate and collaborate with other members of the healthcare team, including physicians, therapists, social workers, and home health aides, to ensure continuity of care.

Document all patient assessments, interventions, and communications accurately and in a timely manner.

Maintain compliance with all regulatory standards, policies, and procedures related to home care services.

Foster a supportive and compassionate environment for patients and their families, promoting dignity, respect, and autonomy.

Participate in quality improvement initiatives and professional development activities to enhance clinical skills and knowledge.

Qualifications:

Valid RN license in the state of Florida.

Minimum of 1 year of nursing experience, preferably in home care or a related setting.

Excellent clinical assessment and critical thinking skills.

Strong communication and interpersonal abilities, with the ability to establish rapport with patients and their families.

Ability to work independently and as part of a multidisciplinary team.

Proficiency in using electronic health record (EHR) systems and other relevant technology.

Current CPR certification.

Reliable transportation and willingness to travel to patients' homes within the designated service area.

Commitment to providing compassionate, patient-centered care.

Flexibility to adapt to changing schedules and patient needs.

Preferred Qualifications:

Certification in Home Health Nursing (HHCN) or similar specialty.
