Performs all responsibilities related to nutritional assessment, recommendations, diet education, and any other duties related to the well-being of in-patient/out-patient population as a remote/telehealth service. Conducts a nutrition assessment for patients, to include inpatients, swing bed rehab or Cancer Center, at nutritional risk per facility policy and procedures. Develops and implements an individualized plan for nutrition intervention based on nutrition assessment and evidence-based standards. Asses educational need and provide nutrition counseling to individuals and groups to include outpatients per MD referral. Collaborate with interdisciplinary team meetings and/or rounds. Experience with electronic charting, the Nutrition Care Process and writing PES statements to provide appropriate documentation for the plan of care. Works cooperatively with food service staff to ensure conformance to diet prescriptions. Demonstrates accountability for the proper use of patients’ protected health information. Ability to perform work independently with minimal guidance. Schedule: up to 24 hours per week Location: Remote or Telehealth EDUCATION: ­ Bachelor’s degree in Nutrition and Dietetics if completion date is prior to January 2024 ­ Master’s degree required to sit for RD exam if completion date is January 2024 or later ­ Accredited specialized training per ADA protocol EXPERIENCE: ­­ Two years experience in diet education ­ Clinical experience preferred LICENSES, CERTIFICATIONS, AND/OR REGISTRATIONS: ­ Registered Dietitian with the Academy of Nutrition and Dietetics and licensed Nutritionist within the state of Alabama EQUIPMENT, TOOLS, WORK AIDS: ­ Computers and other office equipment SPECIALIZED KNOWLEDGE AND SKILLS: ­ Knowledge of medical terminology ­ Comfortable with order writing privileges for enteral and parenteral nutrition, diet orders, and supplements within scope of practice
