About Us:

Hospice of Orleans is a community-based organization with a mission of providing care to with life-limiting illnesses in Orleans County, their caregivers, and their loved ones. We provide our patients and their families with exceptionally personalized nursing care, home health aide, counseling, spiritual and bereavement support. We are currently seeking a Part Time Registered Nurse in our beautiful, eight-bed, Martin-Linsin Hospice Residence in Albion, NY.

Responsibilities:

· Identifies needs and coordinates care and hospice services provided to the patient, family, and caregivers in our Martin Linsin Residence.

· Delivers excellent direct patient care in accordance with the patient’s plan of care.

· Fosters a safe, supportive and compassionate environment to care for patients and their families in accordance with Agency standards.

· Advocates for patients and their families.

· Promotes positive customer relations.

· Maintains harmonious relationship with co-workers, volunteers, consultants, and other providing goods or services to the Residence and in the community.

· Endeavors to fulfill the mission of the Agency.

Expected hours: 37.5 per week
