Description

The Wellness Nurse will be responsible for assisting the Wellness Department and overseeing resident Care .They will be responsible for supervising other nurses, aides, and ancillary staff. The Charge Nurse will also maintain Department of Health Regulations and the processes of the Nursing Department by following Hearth Management’s Policies, Procedures, and Standards while maintaining our Residents’ Rights, safety, and Hearth’s Live More Philosophy.

Requirements

• Receive or make telephone calls regarding resident situations that involve resident care, resident services, or orders from physician offices and Allied Health Providers
• Serve as Charge Nurse for assigned shift and problem solve resident issues to ensure excellence in care delivery while maintaining your work assignment 
• Maintain knowledge of general whereabouts of each resident 
• Monitor residents to identify changes in behavior or appearance 
• Supervise aides, assuring residents are assisted in performing activities of daily living (personal hygiene, medications, activities, meals, etc.), follow up with refusals of care, and assure aides are signing off on care delivery.
• Handling individual resident emergencies up to and including, but not limited to, resident illness, falls, elopement, death, or implementation of the building disaster plan 
• Administering medications to assigned residents 
• Completion of required paperwork up to and including, but not limited to, Incident/Accident Reports; Medication Sheets (MARS); Medication Errors; Narcotic Count; 24-hour Shift Report; documentation in resident’s individual records; implementing new physician orders, change in orders, or clarification of orders; and Post Fall Evaluations 
• Maintain proper staffing levels 
• Assist in maintaining Infection Control Logs, Resident Incident/Accident Logs, and submitting monthly reports to the Clinical Services Department 
• Ensure care needs of residents are met 
• Coordinates with Wellness Director, and Care Leader to ensure quality and continuity of care 
• Follows up as appropriate with supervisor, co-workers, or residents regarding reported complaints, problems, and concerns 
• Follow up on 24-hour Report of any incidents, changes of condition, and/or complaints 
• Maintain Pertinent Charting 
• Assist with potential screens, new admissions, re-admissions from hospital or rehabilitation, or on-going re-evaluations to ensure resident needs are met 
• Updating Care Plans for residents as needed
• Assisting with Case Management meetings as needed
• Investigating any incidents or accidents of residents and follow up on Post Fall Evaluations and interventions 
• Increase care of residents, as needed, through assessments, care planning, and family meetings 
• Provide emergency assistance to anyone within the community 
• Maintain universal precautions, bloodborne pathogens, biohazard precautions, and isolation precautions.
• Facilitate any ancillary provided services, such as physician/nurse practitioner appointments, podiatry services, Certified Home Health Agencies, etc. 
• Notify supervisor/Executive Director of emergency building situations and/or unusual events of abuse and neglect of residents, death of a resident, etc. 
• Maintain communication with pharmacy to obtain delivery of medications and refills 
• Assist in conducting any in-services and department staff meetings, as necessary 
• Provides supervisory support for the Wellness Director, by keeping staff motivated, engaged and providing feedback and redirection when necessary
• Completes any supervisory classes as assigned to increase skill set in managing operations and staff
• Perform other duties as requested

Requirements

Qualifications: 

A valid LPN or RN License, in good standing, from an accredited school of nursing; CPR and First Aid Certification
