The typical candidate is hired below midpoint of the range.

This position will remain open until 04/30/2024.

We are seeking 2 full-time Care Coordinators. The Care Coordinators will work collaboratively with providers and clinical staff to support patients with chronic conditions and/or complex needs in our clinical programs. They will also facilitate communication, coordinate services, address barriers, and promote optimal allocation of resources while balancing clinical quality and cost management. A Care Coordinator will also work in the ambulatory setting, and are members of the provider led interdisciplinary team - they will coordinate care using evidence based clinical guidelines for chronic disease management.

What You'll Need:

  • Bachelor's degree in social work/human services or equivalent preferred, but not required.
  • Experience working as a Certified Medical Assistant preferred
  • Experience working in Healthcare related field required
  • Current BLS, required upon hire
  • Current Driver’s License in good standing and reliable and insured transportation.
  • Care Management Experience, preferred
  • Bi-lingual in Spanish, preferred

What You'll Do:

  • Demonstrate and apply principles of person-centered, strength-based philosophy, motivational interviewing, shared decision making, coaching and adult learning.
  • Demonstrate a sensitivity and responsiveness to a variety of cultural values and beliefs and social determinants of health
  • Respond to provider referrals and/or identify patients who meet established criteria for care management.
  • Completes evaluations with a variety of tools.
  • Conduct basic screenings of patients within primary care, including review of barriers to health self- management; depression; stress; and substance use.
  • Conduct brief interventions and referrals to treatment, supervised by patient's PCP.
  • Provide basic crisis intervention services to patients in need, based on AMSR and ASIST suicide intervention training.
  • Provide basic case management supports regarding health and mental health needs, functional abilities, readiness to change, caregiver stability, social supports, financial resources, environment and safety concerns, life care planning and self-management skills.
  • Help facilitate communication between patients and their providers, and coordinate communication among all of patient's multiple providers.
  • Facilitate development of comprehensive shared plan reflecting patient goals and preferences, self-care, and evidence best practice for chronic conditions.
  • Provide pre-printed educational materials as needed.
  • Provides individual and group health-related education and classes for established patients.
