Summary:
MCIL is seeking a Case Manager to coordinate, implement and monitor comprehensive consumer-directed systems of support and advocacy for persons with disabilities and their families through implementation of case management for effective community integration.
The ideal candidate will be self-directed and have strong problem solving skills. The successful Case Manager will be able to prioritize work, work independently as well as with a team, and will have working knowledge of community resources such as MA, Public Assistance, and Public Housing. This position will be responsible for providing community based referrals and working with consumers to achieve self-direction and community integration.

Social workers must meet the minimum qualifications under Minnesota Merit System
Valid Minnesota Driver's License with acceptable and clean driving record required.

Bilingual (not required, but preferred)

Excellent written and verbal communication skills
Able to work individually and as a team
Collaborate with interdisciplinary members to support a positive, open, respectful communication structure with clients, families, providers, MCIL staff and county representatives.
Computer Skills:
To perform this job successfully, an individual must be competent in computer knowledge including database and internet skills.
