Requirements And PreferencesThe Broward County Board of County Commissioners is seeking qualified candidates for a Human Services Coordinator position.General DescriptionProvides advanced social case management, counseling, and/or coordination of services in an assigned human services program.
Works under general supervision, independently developing work methods and sequences.
Minimum Education And Experience RequirementsRequires a Bachelor's degree from an accredited college or university with major coursework in mental health counseling, marriage and family counseling, counseling education, social work, psychology, gerontology, or other closely related field based upon area of assignment.
Requires one (1) year of direct client social case work/counseling in either an addiction/substance abuse treatment or mental health facility/program, crisis intervention or clinical setting, or in a community resource eligibility center based upon area of assignment.
Special Requirements Depending upon area of assignment, may be required to work nights/weekends/holidays. Special Certifications and LicensesMust possess a valid FL Class E Drivers License at time of appointment and able to obtain and maintain authorization to drive on County business.
Preferences
  • Master's or higher degree in related field
  • Bilingual (English/Creole or English/Spanish)
  • Certified Case Manager (CCMC) or related certification
Duties And ResponsibilitiesThe functions listed below are those that represent the majority of the time spent working in this class. Management may assign additional functions related to the type of work of the job as necessary.
  • Provide holistic and comprehensive case management services (intake, assessment, care and discharge planning).
  • Maintain a caseload; conduct required customer contact and field visits.
  • Open and maintain case records for case management services and tracking, and make recommendations to supervisor concerning specific cases and services.
  • Ensure that all forms are completed accurately. Obtain, review and verify that documents submitted are in compliances with policies and procedures.
  • Advocate on behalf of customers for services, basic needs, and other related issues. Assist customers with accessing/applying for benefits and services.
  • Participate in case staffings and consultations, and in staff meetings.
  • Prepare correspondence, case notes, narrative and technical reports, notifications, and related documents using computer-based applications.
  • Coordinate and deliver agency services.
  • Perform needs and comprehensive assessments; develops or updates the service plans with clients to address their needs and to facilitate referrals to appropriate agencies, service providers, or facilities;
  • Counsel clients, individually or in groups, and/or their families, to facilitate achieving service plan goals, developing life skills, and/or mitigating inappropriate behavior.
  • Perform follow-ups as needed.
  • Ability to use a variety of office software applications, including database, spreadsheet, and/or presentation applications to prepare a variety of documents, forms, and/or related materials.
  • Conduct outreach and networking services, as well as program orientations.
  • Provide prompt, efficient and responsive services and documentation of work completed.
  • Ability to multi-task and prioritize.
  • Performs related work as assigned.
WORK ENVIRONMENTPhysical DemandsPhysical demands refer to the requirements for physical exertion and coordination of limb and body movement.
Performs sedentary work that involves walking or standing some of the time and involves exerting up to 10 pounds of force on a regular and recurring basis or sustained keyboard operations.
Unavoidable Hazards (Work Environment)Unavoidable hazards refer to the job conditions that may lead to injury or health hazards even though precautions have been taken.
Involves routine and frequent exposure to traffic; moving machinery.
