Kabba Incorporated is Hiring an Outreach and Engagement Specialist Near Madison, WI

Position Title: Outreach and Engagement Specialist/Case Manager

Reports to: Clinical Supervisor

FLSA Status: Non-Exempt/hourly; starting at $22.00/hr

Shift: Part-time, Weekends, 4pm-1am and/or 9am-4pm

Purpose statement:

The Outreach and Engagement specialist/Case Manager is a professional who assists with managing client’s cases with a solid background of clinical knowledge and expertise and may be responsible for providing some therapeutic interventions to patients. At times, may collect clinical data, correlate this clinical data, and provide clinical input to the clinical team. Conveys behavioral health and medical criteria and clinical information between providers and team as warranted. May also correlate clinical data for the business office as required.

Essential Functions:

● Work with a team of 3 Clinicians and other staff in providing case management and care coordination for 30 campers living in a city sanctioned encampment and/or provide other homeless services for Kabba Recovery Services.

● Connect clients to mainstream resources and assist with employment, health care and other needs as necessary.

● Ensure confidentiality for our clients, including obtaining releases of information and providing information about rights and responsibilities related to the data of program participants.

● Participation in relevant Kabba Recovery Services meetings as appropriate.

● Other duties as assigned.

● Coordinates in a timely manner, issues or activities relevant to the team, and/or the team’s relationship within the facility or treatment milieu.

● Perform other duties as assigned.

Standard Expectations:

● Complies with organizational policies, procedures, performance improvement initiatives, and maintains organizational and industry policies regarding confidentiality.

● Communicate clearly and effectively to patients and their family members, guests, and other members of the care team.

● Exhibits therapeutic rapport with patients maintaining professional boundaries, being respectful of personal space, giving patients direct attention and while maintaining rational detachment.

● Develops constructive and cooperative working relationships with others and maintains them over time.

● Encourages and builds mutual trust, respect, and cooperation among team members.

Skills and Qualifications:

● CCS certification preferred, or be willing to obtain within 3 months.

● SAC-IT, SAC, or CSAC preferred, or be willing to obtain within 6 months.

● History of working with those living with addiction or mental health issues.

● A passion for ending homelessness and working with people without housing.

● Ability to have a flexible schedule-weekend and evening work required.

● Ability to be trained in HMIS and maintain required certifications, if needed.

● Ability to be trained in Kabba Recovery Services Electronic Health Records system, if needed.

● Commitment to maintaining confidentiality of our program participants.

● Ability to interact with clients in a trauma-informed way that includes harm reduction and centers on the program participants’ needs.

● Ability to meet clients where they are at and be flexible in providing services.

● History of positive working relationships with people of diverse racial/ethnic, socio- economic, and educational backgrounds.

● Demonstrated ability to use creative problem-solving skills to assist persons of diverse backgrounds and abilities, often in extreme personal crises.

● Good organizational skills and ability to work independently and with close attention to details.

● Ability to learn about government or social services agencies that assist people without homes

● Lived experience, a plus.

Job Type: Part-time

Pay: From $22.00 per hour

Expected hours: 16 – 30 per week

Schedule:

  • Crisis intervention: 3 years (Required)
  • clinical: 3 years (Required)

License/Certification:

  • Substance Abuse Counselor certification (Required)
