Qualifications

• Bachelor's degree in a related field
• Athletic Training
• Occupational Therapy Assistant
• Physical Therapy Assistant
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Experience working in a clinical setting
• Familiarity with electronic health records systems
• Excellent organizational and time management skills
• Compassionate and empathetic approach to patient care
• Ability to maintain patient confidentiality

Responsibilities

• Collaborate with our team to develop and implement patient care plans
• Advocate for patient needs and coordinate services to support their treatment goals
• Monitor patient progress and adjust care plans as needed
• Communicate with patients and their families to provide education and support
• Coordinate referrals to specialists and other healthcare providers
• Maintain accurate and up-to-date patient records
• Participate in case conferences and team meetings
• Ensure compliance with healthcare regulations and guidelines
• Provide support and guidance to patients throughout their treatment journey

Job Type: Full-time

Pay: $40,000.00 - $60,000.00 per year
